
Purpose:
The Independent Living Program (ILP) is designed to help individuals who have developmental disabilities achieve their highest level of independence, while at the same time participating in the community. The staff guides the residents in developing social and personal skills in a Christ-centered environment.
The residents in this program provide a natural peer support system, where they help each other accomplish their goals. Each resident maintains a job either through competitive employment or in a sheltered workshop or is involved in a day program.
Recreational activities are planned by the participants and are supervised by the staff. Weekly church attendance is encouraged and daily devotions are offered to promote spiritual growth.
Participants in this program may have their own apartments or chooses to have a roommate. Each person is responsible for the cleanliness of his or her apartment, personal belongings, and household chores (cooking, shopping, etc.).
Provisions:
Since every individual has specific needs, we provide individual-based services. Among other skills, we teach participants how to use public transportation, how to develop a nutritionally sound diet, how to safely use the stove and other cooking equipment, and how to manage their money. We also enroll them in appropriate job-training classes.
Facilities:
Residents in the ILP live in an apartment complex close to a bus line and shopping center that has been approved by the ILP’s board of directors. The residents may choose to have their own apartment or a roommate who is also in the program. Each person is responsible for his or her own personal belongings, household chores and bills. A staff member resides on the premises and is available for help and oversight.
Eligibility:
- Age 18 or over
- Has a developmental disability
- Capable of self-preservation
- Capable of self-medication
- Capable of working or attending a day program
Services:
- Access to routine professional services – dental, medical, etc.
- Assistance in making appointments and obtaining consultation services as needed
- Transportation to events outside the public transportation system, especially during the evening and weekends
- A list of current phone numbers for emergency and advocacy services
- Assistance in planning, purchasing and preparing food
- Monitoring purchase and taking of medication
- Assistance in money management
- Assistance in applying for third party benefits
- Social and recreational activities
- Provisions to assure safety on a 24-hour basis
- Training to increase independence
Fees:
The current fee, set by the board of directors, is $525 per month. The family typically pays this fee. In addition, the participant is responsible for personal expenses such as his or her share of the rent, utilities, telephone, and spending money.
Application:
You should submit the application along with a recent psychological evaluation, a recent photo, and any documents you think might assist us in getting to know your loved one. If the individual meets our criteria, an interview will be conducted with an admissions committee composed of a staff member, the applicant, and a family member. The applicant and family will be notified in writing of the committee’s decision. When an opening becomes available, the approved applicant will enter the program on a six-month probationary period during which CCFH Ministries will determine whether or not the program best serves the applicant’s needs.
